[JSC] Optimize eval code carefully

Reviewed by Yijia Huang.
1. Retrieving baseline CodeBlock and BytecodeIndex in DFG / FTL is
costly as we need to reconstruct them from CallSiteIndex. We should
embed them in JIT code instead.
2. Avoid accessing UnlinkedCodeBlock when unnecessary.
