Set origin-clean when canvas 2D context is reset

Aligns us with Chromium for width & height changes. The HTML standard
only covers this in a non-normative section currently, but that will be
fixed eventually:

Drive-by: update iOS expectations for
security.pattern.fillStyle.sub.html as bug 298431 fixed the flakiness a
while back.
