You are indeed misunderstanding with-flow, and the behavior is not
really inconsistent. :)

Let me try to explain.

The big picture problem is that you're modifying children in the
rendering phase. This does have its application, but especially as a
beginner this means inviting lots of trouble with seemingly
inconsistent results. That's because you have to be very specific as
to what happens when.

Usually it's better to use with-flow, do-widget and other forms that
change the widget tree in an action (e.g. via render-link).

The small picture is this:

1. The old state (before the with-flow) is rendered
2. During this rendering, the widget tree is changed by your with-flow
form. However you don't see this change yet since.
3. When you revisit the menu item you see the new state, as expected.
