Hi, don't you have in the instance 99 log something like : "Exception while sending response: java.net.SocketException: Broken pipe" ?
Adaptor is marking instance 99 dead cause it doesn't reply in time, it close TCP connexion and resend the request to another instance. As a result when the response is done the instance can't send it back to the adaptor cause it hasn't wait enought and you have in the log the "Broken pipe" exception.
