Looking for insight into the use of this questionnaire...do others
really see this as a tool they can use on a application by application
basis (one for their EMR, one for the Trauma Registery, one for the PACS
system) or is it really meant for an overall/overreaching survey of the
actual Covered Entity Information Technology system?

For example question 6.1.5 - Are mechanisms in place for holding users
responsible for their actions? (Everyone signs the same type of
Information Technology use agreement. Why would this differ by app?)

Or

Question 6.1.6 - Are hiriing, transfer and termination procedures
established? (Again - this would be an overall function, don't see it
differing by app.)

However

Question 6.1.4 - Is there a process of requesting, establishing,
issuing and closing user accounts? (Depending on whether your entity is
highly IT centralized or de-centralized, this could differ by app.)

Information on actual experience with actually using this long and
detailed survey would be appreciated!!!! MIMI
