We have found the same results, NICs do not always auto negociate the duplexing correctly.  So what we have done is hardcode server nics and their switch ports 100/full and for the floor switches where the clients are we hard code those to 100/half and let the clients remain auto/auto. All the testing we have done shows that NICs will correctly select half duplex if its being told that, its going to full that presents the problem.

WE have found that the bandwidth usage of clients is easily met with 100/half and for the very few users that need 100/full we can hard code that setting on both their PC and switch port.
