I have come across this info, that might be a clue why this was happening
sometimes:

I think I may have had templates structured such that wiki code ended up being
embedded in HTML, or something like that. I'm going to close this as WONTFIX
for now, until the issue pops up again and I can test if Tidy resolves it.
