Doing the query on page save would likely be less load and if a schema change
was done, it would be the only way; populating it on-demand would just be kind
of odd and hard to do. If only 1 in 5 edits is to an unwatched page, you'd have
to load the data for at least 250 to be able to hopefully generate a list of
50.
Counting only active editors would be possible, but it might add too much to
the query to be usable.
