I see the scoping problem, on the other hand, it would be overkill to require two containers for a simple Wikibase deployment in which I am guessing the regular Wiki part would also be used.

Given a separate parsoid container would make the configuration of visual editor relatively simple though.

Indeed parsoid should be a separate container.

As for adding custom extensions, the bundle images probably won't include ALL of the possible extensions, only the key ones.

You might want to give https://addshore.com/2018/06/customizing-wikibase-config-in-the-docker-compose-example/ a read about shoehorning in custom extensions. The last section of that blog post also talks about making your own Dockerfile which would probably be the correct way forward.
