Most of the time spammers actually have your address, although
sometimes they resort to dictionary type attacks.
Email gets transported IAW the delivery information on the SMTP 'envelope'.
Once it's delivered to your ISP, the envelope is discarded.
Common email clients always set the delivery address equal to the
'To:' address except when using 'BCC:'.  Spammer email programs
are quite sophisticated, but the delivery address is set similar to the
'BCC:' mechanism.
