Just wanted to add a couple more things to be aware of. The csv files should not have " " around the values. They should be:
first,last,address,city,state,zip for the desktop geocoder and: first,last,address,city,state,zip,lat,lon,up_rate,dn_rate,tech_code for the upload file. XL and Open office try to export to CSV with " " around the fields unless you specify not to have them. Also, if you are submitting pre-geocoded data, make sure that before exporting your csv from a spreadsheet to format the lat, lon columns to be a decimal with 5 places after the decimal point. I know open office cuts off the last 3 places in the default format so that lat/lons came out like 42.35 and -96.21. This is not enough accuracy for proper tract referencing.
