The document was designed as an internal reference so the broken link
was not intentional.

I've posted a working copy:

It's a sample database, menu list and set of forms you can play around
with.

You should be able to use FormEdit to build your own table, then define
a form for it. Once the form is built you can use admin.taf to add a new
item to the menu. Look at the existing example for "Mutual Funds" for
the formatting of the querystring for the menu definition and the form
definition inside FormEdit. The table editor will probably barf if you
try to modify a table, so try to build one with all the fields you need
for testing. 

Don't worry if you break it, it's only a sample database.
