Hello,
I created a very simple form whose source code is
at:http://paste.ubuntu.com/610489/

 and header file at: http://paste.ubuntu.com/610492/

After issuing MAKE I get the errors specified at:
http://paste.ubuntu.com/610484/

Please let me know what is the problem with my source code so that I
can correct this error.
