Basically, this patch splits the current '0.95.X' section into sections for each versions that have been released, and take opportunity to add a few missing info. I have changed the '0.95.X' for future changes into '0.95.6', which might end up being '0.96.0', the reason for not keeping 'X' is that when updating things it may get people think about it and add the appropriate section.
