It is possible that issues created before I set up the workflow were in an incorrect state. I just worked through an issue and it seemed fine. I've changed the name of the final step (i.e. from closed to resolved) to "verify" (previously "approve") which makes it clearer I think. Ive also added a "reopen" step here that allows the reviewer to reject a fix.
