I've fixed the links to the icon file within the wiki pages (which all seem to need to point to the "creating a " page.
And I've escaped the example link tag on the "creating a " page, which I suspect was being turned into a real one by MediaWiki. But the url http://codex.wordpress.org/favicon.ico points to a slightly broken 301 handler: the content returned says The document has been permanently moved to <A HREF="%s">here</A>.<hr /> where %s has not been replaced. It seems to be a standard litespeed handler page. The headers returned do include location header of http://s.wordpress.org/favicon.ico which does return the ico file.
