I'm getting the following JS error when trying to add a new output mapping to a
data service based on MS excel or CSV.
document.getElementById("txtDataServiceSQL") has no properties

Steps to reproduce:
================
1. Start to create a data service by selecting MS Excel as data source
2. In the second step of the wizard, click on 'New Query'
3. Enter data extraction parameters for excel
4. Enter grouped by element and row name
5. Click on 'Add New output mapping'
