I think the above can be decyphered as:

     (%flow;)* -(A|%formctrl;|FORM|FIELDSET)
     -- push button -->

opening and closing tags are required, it can contain any number of
flow elements (block or inline), but can't CONTAIN links, other form
controls, forms or fieldsets.
