The options on the monitor page only apply to the behaviour while
offline.  If you are offline when one of the specified times occur
then next time that you go online and fetch pages it will be fetched.

Actually that is not totally true.  If you are online when one of the
times occurs then the page will be fetched next time you are online.
But only if you didn't get the page while online after the specified
time.

If you are online for more than an hour it does not mean that the page
will be fetched twice.
