The indicators are there to show the state of the cache for the URL.
It is not intended to show what would happen when going online and
fetching, only the current state.  The 'X' is there to explain why the
URL is not cached and why there is no link to it.

There are just too many possible states that could be shown
(considering all of the options in the configuration file) to be able
to show all of them.

Since this isn't a page that we look at every day it is OK for the
user to need to do further exploration for each URL to find out all of
the conditions that will apply when trying to fetch the URL.

The '+' is the state of the URL in the cache, not the state of the
images option in the configuration file.  You might have disabled
fetching images today, but it is still cached and nothing is going to
change that.
