I sort of stumbled upon this and I'm not sure what the root cause is but the 
result is an unhandled exception.  It seems to be some weird combination of 
xsl:output's cdata-section-elements and using a xsl:value-of in a xsl:comment.  
The input xml doesn't seem to affect it, but if I change one of these three 
there is no problem, I'm attaching the problem stylesheet.
