I suspect this is more an anaconda/kernel issue, I've seen this on other
(desktop) systems occasionally when changing partitions on a drive and
the kernel is determined to hang on to the old partition table until its
rebooted.

Mark: had there been a previous OS install on this node?

I've seen some mentions that having a previous LVM config can trip up
Anaconda at times and so I've seen someone clean that with vgremove with
a comment that quotes the error message you mention.
