Description:
1. The xtags DTD allows default values to be specified for parameters, but these only
get listed in the generated documentation if they are part of an option set.
Non-option defaults should be printed too.
2. Applicability conditions can be specified at both namespace and tag level, but
aren't displayed anywhere in the generated documentation, only the ones at the tag
parameter level. The condition-description elements at the higher levels ought to be
printed too.
