Description:
Current xdt code that generates EJBRef for ibm-web-bnd.xmi uses       
<XDtClass:forAllClassTags tagName="web:ejb-ref">. This causes duplicate EJBRef 
to be generated.

This is quite a serious problem as when EJB is being lookedup via its logical 
name, it can return the wrong stub.

Perhaps it should use <XDtWeb:forAllEjbRefs> as implemented in web_xml.xdt
