Check out the generators at http://www.dentaku.org.  I've got an X2 plugin
for generating Struts actions from UML, it would be trivial for me to add
Struts forms if you were interested in using UML for your source notation.  

If you do want those, make a post to the dentaku-user mailing list!  :-)
