I've reproduced procedure described at:-

twice and both time succeeded.

Once on remote Ubuntu HH dessktop host,
once on Ubuntu HH HVM at Xen 3.2.1 CentOS 5.2 Dom0.
It really doesn't matter, where you prepare image.
When done just "scp" it along with swap file
to SNV(OpenSolaris) Dom0 or any other Xen 3.X Linux Dom0.
It should work anyway.

1.# mkfs.ext3 /var/tmp/ubuntupvm.img
2.# mkswap /var/tmp/ubuntupvm-swap.img 
3.# mount -o loop /var/tmp/ubuntupvm.img /mnt

All looking good?

# df -h /mnt
Filesystem            Size Used Avail Use% Mounted on
/var/tmp/ubuntupvm.img           20G  173M   19G   1% /mnt

4.# apt-get install debootstrap
5.# debootstrap --arch=amd64 --components=main,updates,universe,multiverse 
hardy /mnt \
  http://ftp.iinet.net.au/pub/ubuntu

It might take 1-2 hr dependent on network connection.
Repository been selected was :-
http://ftp.iinet.net.au/pub/ubuntu
it might not work at mean time, then any other for
Ubuntu HH would be good if extraction phase committed
properly. No extraction errors is a must

When debootstrap extraction to /mnt was done ran:-

6.# chroot /mnt
7.# export LANG=C

and so on .. Follow the link above.

Creation Ubuntu (Debian) images and "scp" them to 
Xen 3.X Linux( or SNV) Dom0 is universal procedure 
not dependent on OS (Nevada,CentOS,Fedora,Ubuntu) 
running at target Dom0.

Of course the "chroot /mnt" command does not work either
