I detached the instance and schema document that were attached to this report, 
changed their encoding decls from UTF-16 to UTF-8, ran 

java dom.Counter -v -s -f 18372.xml

and received the message:  

[Error] 18372.xml:9:21: Duplicate unique value [ID Value:  A2] declared for 
identity constraint of element "doc".
18372.xml: 1161;10;0 ms (3 elems, 5 attrs, 0 spaces, 6 chars)

I don't know what to suggest, except that the fact that I had to alter the 
encoding decl suggests the code you're running isn't simply reading this off 
disk; so perhaps there's something wrong there.
