XInclude processing not working properly

I think I have seen this issue myself: it stems from the assumption that the 
arrays fSawFallback, fSawInclude and fState are the same length.  While working 
on the XPointer implementation, I have seen cases where this assumption was not 
correct.  Perhaps a fix for this bug might simply be to initialize the three 
arrays separately.
