[PATCH] Empty type definitions and block or final

This patch addresses two minor issues:

The first is to emit null type definitions instead of ignoring them.
For example, if a particular simpleTypeDefinition is null, the following
output should be produced:
<psv:simpleTypeDefinition xsi:nil="true"/>

The second is more cosmetic than anything. It removes unnecessary spacing
from block or final strings in translateBlockOrFinal.
