Avi: I got the same output as you using w32tex (XeTeX 0.9997.0 with ICU 4.4.1), the problem is that the manual shows meteg in medial position, not to the left of the hataf patah.
It appears the problem is probably with ICU. I've attached a picture of the sequence in Notepad, which displays it correctly. -Andy
