I have recovered the reencoding of pdf strings, since we don't have
right bookmarks without the hyperref package.
The destination in pdf:dest is also reencoded. Thus
/D<feff00e4006e0063006800f80072>
and
/Names[<feff00e4006e0063006800f80072>7 0 R]
in Heiko's exapmle.

Thanks,
Akira
