You are running the "nv" driver that came with XFree86
but you have the GLX from NVIDIA's binary drivers installed.
Remove NVIDIA's GLX (/usr/X11R6/lib/modules/extensions/libglx.so)
so that the GLX that comes with XFree86 can get used.
