- remove unnecessary blank lines and spaces between pcode constants
to produce more compact output

! include <OciDL.h> for __MINGW32__, __DMC__ and __WATCOMC__
+ declaration of IID_IClassFactory2 for OpenWatcom (actually this should
have been defined in uuid.lib packed with OpenWatcom binaries, but the
current default CVS compiler switches make it invisible. This is most
probably due to different compiler flags -> stacks vs registers <?>)
