I am a programmer so I would be able to modify whatever style sheet
changes that would be required. Perhaps you can point me to which file
in the xml editor or FO Converter XSL Util contains header/footer
information? Ultimately I want the PDF output file have a custom page
header/footer. I'm pretty new to docbook and your tools.

Sorry but this cannot be done from within XMLmind XML Editor. You need
to customize the DocBook XSL style sheets for that. See
