Yes, this could be the problem[***], but it could also be something else
(e.g. entity references).

Sure. Please edit XXE_install_dir/addon/config/docbook.xxe using text
editor and replace these lines:
<!--
<include location="xxe3_compat.incl" />
-->
by just this line:
<include location="xxe3_compat.incl" />
(In other words, ``uncomment out'' the <include> line.)
You'll have to do this each time you'll upgrade XMLmind XML Editor.
