I've been thinking about how best to do this in the absence of native HTML
support, and you can see my tests for select1 here:

I think example 4 best matches what is required, though perhaps "*other*"
is not the ideal choice of label. However, it seems to need little extra
support, and does what it is meant to.
I realise <select/> is more work, and I shall continue to think about that
case.
