Mozilla Firefox does not handle namespaces correctly in its bugged XSLT processor.
Adding such dummy attributes allows XSLTForms to detect namespaces correctly anyway...

It took me ages to get to grips with namespaces and I ended up with this
'approach' (see attached) which works for my use cases, but I use [old]
xhtml forms. Whether the OP's example is something to do with namespaces,
or the document format I've no idea.
The attached works on my setup. It is the OP example altered to fit the
format I use.
