We really wish we knew. So far the only condition we can reliably reproduce
is that it never happens to an engineer on the X team, which is making it
damned hard for us to figure out why other people hit it so often.
Unfortunately, the budget doesn't allow us to hire all of you so it stops
hitting you too...
