Confirming bug as User Error. Apologies. It turned out that when accessing the
web page through the firefox browser on my VNC session, the image choices drop
down is missing, but sitting at the machine itself, the image appears.

Bug description:
The glance database images table indicates 3 images, and horizon does believe
there is a choice because it displays "select image"
and on the admin images page, all images are listed in table.
