Just a few extra points.

I'm not sure if we need the Fl::lock / Fl::unlock in MiscGui.cpp
It seems to work without them, but I've not tried to test it extensively.

The Fl::lock in main and LV2 do seem to be necessary. Curiously, there's no
Fl::unlock at termination (and never was) so I don't know if that is assumed by
FLTK on exit, or if there is supposed to be something there.

As an aside, from main.cpp the GUI is linked to each instance via GuiThreadMsg
in Config.cpp I don't know why this couldn't have been used for LV2 but Andrew
did the work for both of these, so there must be a reason!
