Boy, does that look painful. Although I understand the intention, the results 
aren't actually grammatical.

There's the comma splice in the title text: "Do you utilize an escrow account, 
if so an account must be specified..."

Then, adding the tail end for the various options results in gibberish: "... if 
so an account must be specified ... pay taxes"-- doesn't make sense. 

So this is just a fail on all levels. 

It would be preferable if this were rewritten to use more formal language. 

Like:

Title: Escrow Account Settings

Content
If your loan utilizes an escrow account, enter the necessary information here. 

Use Escrow Account
          Escrow account 
Escrow includes Taxes
Escrow includes Insurance
Escrow includes PMI
Escrow includes other expenses

I'll admit I have never used this assistant, so I don't know why each option 
includes a secondary check box, or what effect the choices have, but I'll leave 
that to others.
