I would highly recommend you restructure your table to

Your query would simply display all records where whatever your response field isn't null.  I have dealt with path you are attempting to go down, and it isn't pretty.  the table I had to deal with had comment1 .... comment8.  This table format will only lead to pain and suffering. 

Each question and response should be it's own record.  This format would enable you to track unanswered questions and only select those questions which were answered.

There may or may not be data in any of these fields depending on
what the respondent felt toward the options given.  I have the
feeling I'm not going to be able to accomodate this on the report,
what I WANT is if Field 1, 4, 6, 8 and 9 have data, then only the
data and the labels for the data print.
