Dear sir,
I have faced issue on "Place of Birth" certificate. because before
1899, there was no mandatory on taking birth certificate of the child.
since, i have not holding the same, the police officials have asked me
to give notary affidavit. later, i have overcome this problem.

i would suggest you that if they are really asking your disability ID
then, you may show your UDID (if you have) or otherwise, you can
explain the exact fact about your situation and show  the FIR copy.
the passport office has facilitated me for completing formalities as
quick as possible, since, i have specified that i am a disabled.
