Application and Disability certificate has to be verified by the CMOs office. Once done, the UDID card can be downloaded.

They will verify the application. If you have a valid Disability certificate then you should get the UDID number and card within ten days. If you do not have a Disability certificate then you would be referred to the Ophthalmologist who will examine you and issue the necessary document. You could then take that to the CMOs office and your job will be done. The card once ready can be downloaded from the portal. Do let me know how it goes.
