In ACPI mode, the system runs in IOAPIC mode, but
with "acpi=off" and "pci=noacpi", the system runs in XT-PIC mode.

Please boot in ACPI mode with just "noapic" and report the /proc/interrupts
and report if the ethernet works then.  If it does, then perhaps this is
an interrupt configuration bug, or something specific to the IOAPIC.
If it does not, and the interrupt configuration matches the "acpi=off"
or "pci=noacpi" or "acpi=noirq" case, then the cause is somewhere else.
