Hi, Jennifer -

At UAB, new employees are required to attest to the code of conduct on a 
one-time basis within 60 days of hire.  This is completed and tracked through 
our employee learning management system.  Completion rates are included as one 
of the metrics on our annual compliance scorecard.  Happy to discuss how this 
works here further, if you are interested.

I am doing a review of our institution's Code of Conduct and was wondering if 
your institutions require employees to attest to or acknowledge the 
institution's Code of Conduct. If yes, how often is it required (one time or 
recurring) and how is it obtained (manual or electronic database platform)?
