OK group, my turn.  This goes back to 1986.
 
I had just purchased my first camcorder (anyone remember the consumer versions 
of the Sony BetaCam?).  I was asked to shoot video of a friend's wedding.  
Lucky for me, this was  freebie job.
Taking video of the ceremony went well, but in all the excitement of the post 
ceremony activities at the church, I, too, lost count of the evil "on-off" 
record button.  When the festivities were over, I thought I turned my camera 
off and put it nicely away in it's hard case.  When I opened it up (1/2 hour 
later) at the reception, I noticed it still on.  Other than a partially drained 
battery, I saw no real dilemma.
 
When I played back the tape for editing, I noticed that I had footage of me 
putting my camera in the case, closing the case, and 1/2 hour of completely 
black video. (And I didn't even charge my friends for that 'superior' footage).

Here's my dumbest thing:

the awesome colors) I hired a model to make my first porn flick.

As you all know, the stop/start recording switch on most camcorders works 
as a toggle.

Somehow I lost the feel with the toggle, and each time I was supposed to 
stop the camcorder, I made it start recording, and then when I was finished 
giving the model instructions for the next shot, I accidentally made it 
stop recording.

You can all guess what the end result looked like. It was 100% "behind the 
scene" footage.

Luckily the model was an amateur and quite cheap.

In the next few years this happened again a few times, the problem with the 
toggle switch. For me, a camcorder with separate "start recording" and 
"stop" buttons would be better. Now I always double check if the camcorder 
is recording when recording something really unique and important.
