Hi Reed,

I'm not aware of any issues with certain ad types not be included in
the reports.  My initial thought is that some campaigns may be using
criteria beyond keywords and managed placements, such as automatic
placements, audiences (user lists), etc.  Clicks, impressions and cost
from these types of criteria wouldn't be included in the keyword and
managed placements reports but would in the account report.  Have you
looked into this?
