Was replacing dejavu rather than deja, but have now tried both. As yours therefore, except that I haven't deleted all the other entries. Cannot recreate in either of the OOo versions mentioned. Entry is still there if I close Writer and restart. Are you entering Tools>Autocorrect with the same language (see top of dialogue box)? (If you create the entry in English (UK) but come in from a document formatted as language English (USA), the entry will appear to have disappeared.
