If we use "Esci" we reproduce the reason this issue has been started. Esci is ambiguous and is not clear that the file will be not saved. But the question is "Do you want to save the changes?" so the answer should really be "Yes" "No" and "Cancel", translated in "Sì", "No", "Annulla". So I suggest to change the English answers as first sanity goal (this way user have to thing twice... reading the question and trying to "map" the given answers into a "yes/no/go back" actions).
